I HEREBY ASSUME ALL RISKS OF PARTICIPATING IN ACTIVITIES AT PKB PILATES, including but not limited to risks arising from negligence or carelessness on the part of the studio, instructors, building owners, or other participants; risks related to the use of equipment; and any other risks that may result in injury, illness, disability, or death.

I certify that I am physically fit and in good health to participate in Pilates and related activities. I acknowledge that it is my responsibility to consult with a physician before beginning any new exercise program, including Pilates, to ensure that I am medically cleared for participation. I understand that Pilates involves physical movement that may not be suitable for everyone and that participation is at my own risk.

I acknowledge that this Accident Waiver and Release of Liability applies to all activities, classes, training sessions, workshops, and events at PKB Pilates and governs my actions and responsibilities in those activities.

In consideration of being allowed to participate in activities at PKB Pilates, I, on behalf of myself, my heirs, executors, administrators, next of kin, successors, and assigns, hereby agree to the following:

1. I WAIVE, RELEASE, AND DISCHARGE PKB Pilates, its owners, instructors, employees, contractors, agents, volunteers, and the owners of the building where the studio is located from any and all liability and responsibility for injuries, illnesses, accidents, natural disasters, or other acts of God incurred during participation in any class, private instruction, or event.
2. I WAIVE, RELEASE, AND DISCHARGE all liability, including but not limited to personal injury, property damage, property theft, disability, or death, whether occurring at the studio, while traveling to and from the studio, or during off-site studio-related events.
3. I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE PKB Pilates, its instructors, employees, volunteers, agents, and the owners of the building from any and all liabilities or claims arising from my participation, whether caused by negligence or otherwise.

I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the applicable jurisdiction, and that if any portion is held invalid, the remainder will continue in full legal force and effect.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, FULLY UNDERSTAND ITS CONTENTS, AND SIGN IT VOLUNTARILY AND OF MY OWN FREE WILL.
